By booking or participating in a tattoo or related service from liz Venom you hereby agree to the following Terms & Conditions.

By booking an appointment you acknowledge that you have read, understand and agree to be bound by these Terms. If you make a booking on behalf of other participants, you guarantee that you have the authority to accept and do accept these Terms on behalf of the other participants in your party.

1. THE BOOKING CONTRACT

Your booking is confirmed and a contract exists when your artist, piercer or authorized agent issues a written confirmation after receipt of the applicable (non-refundable) deposit amount.

Please check your confirmation carefully and report any incorrect or incomplete information to your artist, piercer or authorized agent immediately. 

You must be at least 18 years of age to make a booking for a tattoo. Failure to be of the appropriate age at the time of your appointment, will result in the cancellation of your appointment, and forfeiture of your deposit. 

You agree to provide full, complete and accurate information to Bombshell tattoo Galerie. 

2. BOOKING ON BEHALF OF OTHERS

By booking on behalf of other participants (example: a group booking) you are deemed to be the designated contact person for every participant included on that booking. This means that you are responsible for making all payments due in connection with your booking, notifying the artist, piercer or agent if any changes or cancellations are required and keeping your party informed.

By booking on behalf of another person or persons, you represent and warrant that you have obtained all required consents and verified the age of those involved. You are responsible for verifying that any information you provide on behalf of another participant is complete and accurate and that Bombshell tattoo galerie will under no circumstances be liable for any errors or omissions in the information provided to complete a booking.

3. REQUIRED MEDICAL INFORMATION

You must provide any medical information reasonably requested by Liz Venom and may be required to complete a waiver form including medical information that is relevant for the purpose of receiving a tattoo, piercing or related service. 

Medical Forms are mandatory for any procedure that involve breaking the skin barrier.

In making a booking you agree to complete the Medical Form honestly and to disclose all relevant medical information accurately and fully.

Liz Venom will maintain all information collected in accordance with our privacy policy as listed on this website.

Liz Venom reserves the right to request further information or professional medical opinions where necessary, as determined in its discretion, for your safety or the safety of our staff and additional clientele. 

Liz Venom reserves the right to deny you permission to receive a tattoo or other service at any time and at your own risk and expense where the artist, piercer or agent determines that your physical or mental condition renders you unfit to be the recipient of that service or you represent a danger to yourself or others.

Pregnancy is considered a medical condition and must be disclosed to Liz at the time of booking. 

Liz does not offer services to pregnant, or breastfeeding individuals. 

In the event that you do not complete the required Medical waiver form or provide medical information reasonably required by the Artist, piercer or agent for any reason Liz Venom reserves the right to cancel your booking and all applicable cancellation fees will apply.

You are responsible for assessing whether a tattoo or associated service is suitable for you. You should consult your physician to confirm your suitability to participate in any of the planned activities. You should seek your physician’s advice on vaccinations and medical precautions. Liz Venom does not provide medical advice.

It is your responsibility to assess the risks and requirements of each aspect of receiving a tattoo, piercing or related service based on your own unique circumstances, limitations, fitness level and medical requirements.

4. SPECIAL REQUIREMENTS

Any special requirements must be disclosed to your artist, piercer or service agent at the time of booking. Liz Venom will use reasonable efforts to accommodate special requirements or requests but this is not always possible given the nature and physical / artistic limitations of permanent body modification. Certain procedures may be inaccessible to you if your mobility or physical body shape is limited in any way.  
As Liz Venoms primary place of business, Bombshell Tattoo Galerie, is located upstairs, should you be mobility limited and require special accommodation this must be disclosed to your artist, piercer or service agent prior to booking so that we may arrange an alternate location for your service. 

Liz Venom will not be held responsible for any fees or travel expenses or injuries you incur as a result of your of failure to disclose a special requirement or physical limitation.

 

5. AGE REQUIREMENTS

Anyone under the age of 18 at the time of their booking is considered to be a minor. Minors are not allowed to attend tattoo appointments at any time.

6. DEPOSITS

All deposits are 100% non refundable.

At the time of booking, a non-refundable deposit to the value specified by your artist, piercer or service agent is due to the person whom you booked with. 

Should you cancel the booking entirely, or reschedule within 72 hours of your appointment time, your deposit will be forfeited and become the property of the artist. 

Deposits are non transferrable, and are only valid for the tattoo, piercing or service that was confirmed at the time of your booking. 

Should you change the agreed upon artwork, placement or service a new deposit may be required, at the sole discretion of Liz Venom.

Should you require to reschedule your appointment you may do so a total of 2 (two) times per appointment allowing that 72 hours notice was provided each time a reschedule is made. 

Deposits will be forfeited to Liz Venom should you reschedule 3 or more times, or with insufficient notice (less than 72 hours notice) 

7. DETAILS REQUIRED FOR BOOKING

As a condition of booking, you must provide the information (such as reference photos or artwork subject matter clarification) requested by Liz. 

If you fail to supply information required you will be liable for any costs, fees or losses including failure to have artwork ready on time, and therefor the inability to proceed with a booking.

In the event that you fail to supply information required by artist, piercer or service agent, that artist, piercer or service agent also reserves the right to treat your booking (or the relevant component of your booking) as cancelled and levy any cancellation fees deemed reasonable by Liz Venom, at her sole discretion. The information required by the artist will vary depending on the service and will be communicated to you or to your authorised agent during the booking process. 

Liz Venom will not be held responsible for any fees or travel expenses you incur as a result of errors, omissions, inaccuracies, late, misplaced or otherwise incomplete information you have provided.

8. CANCELLATION BY THE CLIENT

You may cancel your booking by notifying Liz Venom via the method in which you booked.

Cancellation fees or the forfeit of your deposit, if any, will be determined with reference to the date on which notice of cancellation is received by the artist.

Cancellations made outside of standard (office) business hours, will be considered received at the start of the next business day. 

Cancellation of a booking:
• Cancellation received with more than 72 hours notice before the tattoo or piercing service will result in that deposit will be held by the artist in accordance with these Terms for a period of 90 days. Should the you not contact your artist, piercer or service agent within 90 days to rebook the deposit will be considered abandoned and therefor will become the property of Liz Venom. 
• Cancellation received with less than 72 hours notice before the tattoo or piercing service will result in that deposit being forfeited in full, and that deposit will become the property of Liz Venom

 

9. GUARANTEE OF SERVICE AND CANCELLATION BY LIZ VENOM

A tattoo or related service appointment will become a guarantee when at least one booking is secured by a valid deposit.

Liz Venom guarantees that all scheduled appointments booked and secured with a valid deposit will proceed as indicated on the applicable confirmation, subject to reasonable itinerary changes as described in these Terms or good faith health and safety concerns. 

If a service is cancelled by Liz Venom more than 72 hours before the date of your booking for reasons other than Force Majeure or as a result of a suspension of operations by the studio from which she is working and the cancellation is not caused by your fault or negligence, you will have the choice of accepting from your artist, piercer or service agent:
(a) a substitute appointment date
(b) a full refund of all monies paid for the cancelled booking.

If a service is cancelled by Liz Venom less than 72 hours before the date of your booking for any reason other than due to Force Majeure or as a result of a suspension of operations by the studio and the cancellation is not caused by your fault or negligence, the Artist, piercer or service agent can offer you either: 
(a) a substitute appointment date with a discount to the value of your deposit
(b) a full refund of all monies paid for the cancelled booking.

If a cancellation at any time is due to Force Majeure or a suspension of operations by the studio at which your appointment was confirmed to take place occurs you will be offered a new appointment time.

Liz Venom is not responsible for any incidental expenses or consequential losses that you incur as a result of the cancelled booking including visas, vaccinations, non-refundable flights or rail, non refundable car parking or other fees, loss of earnings, or loss of enjoyment.

10. LIABILITY

Liz Venom and her respected affiliates, officers, directors, successors, representatives and renters shall not be held liable for any damage to, or loss of, property or injury to, or death of, persons occasioned directly or indirectly by persons engaging in body modification including but not limited to tattoos, piercing and associated services. 

11. IMAGES AND MARKETING

You agree that, while participating in any tattoo or service, images, photos or videos may be taken by your artist, other artists at the studio or other clients, that may contain or feature you or your likeness. You consent to any such pictures being taken and grant a perpetual, royalty-free, worldwide, irrevocable license to Liz Venom and assigns, to reproduce for any purpose whatsoever (including marketing, promotions and the creation of promotional materials by or with sub-licensees), in any medium whatsoever, whether currently known or hereinafter devised, without any further obligation or compensation payable to you.

12. PRIVACY POLICY

By submitting any personal information to Liz Venom you agree to the privacy policy listed on this website.

13. AMENDMENTS

Liz Venom reserves the right to update or alter these Terms at any time, and will post the amended Terms on this website.

Any amendment will take effect 10 days after being posted to this website.
